Independent Contractor Application
To start the process simply click the "APPLY HERE" button and you will be prompted to complete a brief application questionnaire. A new window will open and you will be required to complete all requested fields as well as upload a clear picture of your photo ID, SSN proof, and personal head shot photo for your agent badge.

Independent Contractor Agreement
Once you complete STEP 1 you will be contacted by an on-boarding specialist. If we proceed, you will need to proceed with Step 2. You will need to return here and click "SIGN HERE" button below to complete Step 2 IC Onboarding Pack and submit. BE SURE TO ENTER YOUR LEGAL NAME AND SIGN YOUR NAME AS IT APPEARS ON YOUR GOVERNMENT-ISSUED PHOTO ID.

Campaign On-boarding & RAD ID
Once you have completed STEP 1 & Step 2 above you will need to return here and click the "CLICK HERE" button below to proceed with Step 3 requesting to On-board the Campaign. NOTE: To participate in the Lifeline or ACP campaigns you will need an active RAD ID. To establish or retrieve your RAD ID please click the link below. IF you already have your RAD ID please submit it in the email along with your legal first and last name and your mobile phone number
